Refractive Lens Exchange Surgery in San Francisco & Bay Area

If you struggle with nearsightedness, farsightedness, or astigmatism, Refractive Lens Exchange surgery can restore your vision and give you freedom from glasses or prescription contacts. This procedure is also suitable for those with existing cataracts, or as a preventive measure against age-related cataracts. It can also correct presbyopia, an age-related condition that makes it difficult to see up close.

What Is Refractive Lens Exchange (RLE)?

RLE vision correction is a surgical alternative to LASIK and other laser eye surgeries. It involves replacing the eye’s natural lens with an artificial intraocular lens (IOL), effectively providing a permanent implanted lens to help you see clearly.

RLE corrects poor vision, stabilizes eyesight, and helps prevent age-related conditions such as cataracts and presbyopia. The procedure uses the same technique as cataract surgery, with one key difference: the lens is replaced proactively, before a dense cataract has the chance to develop. It is one of the most performed and successful ocular surgeries in the world.

How Refractive Lens Exchange Surgery Works

In preparation for RLE surgery, your eye doctor will thoroughly examine and scan your eyes to confirm you are a suitable candidate and determine which type of intraocular lens will best meet your needs.

The RLE operation is an outpatient procedure performed under local anesthetic, applied either as an injection behind the eye or through medicated eye drops. If needed, patients can also receive a mild sedative to help them stay relaxed throughout. A spring clip holds the eyelids open, and a sterile adhesive covering keeps eyelashes clear of the surgical field.

During the procedure, you will be comfortably reclined while your surgeon works under a microscope. Surgery typically takes around 20 minutes and involves the following steps:

  1. A small incision is made at the junction of the white of the eye and the cornea.
  2. The natural lens is broken up using a high-frequency vibrating probe, then removed with gentle suction.
  3. The artificial IOL is carefully inserted, unfolded, and positioned correctly within the eye.
  4. A supportive gel used during implantation is flushed out and replaced with sterile fluid and antibiotics.

You should feel only mild pressure during the RLE operation. Depending on your preferences or your surgeon’s recommendation, both eyes may be treated in the same session.

Benefits of RLE Vision Correction

RLE surgery offers a range of significant advantages for the right candidates:

  • Restores Vision at All Distances: Advanced intraocular lenses can correct vision across multiple focal ranges, reducing or eliminating the need for prescription glasses or contact lenses.
  • Suitable Where LASIK Is Not an Option: RLE provides a clear pathway for those over 50, individuals with age-related vision changes, or patients with dry eyes who are not eligible for laser eye surgery.
  • Permanently Eliminates Cataract Risk: Because the natural lens is removed and replaced, the risk of developing cataracts in the future is completely eliminated, removing the likelihood of needing additional eye surgery later in life.
  • Fast Recovery: The procedure is minimally invasive. Most patients can resume normal daily activities within a few days and notice meaningful improvements in vision during this period.
  • Lifelong Results: Unlike some vision correction procedures that may require adjustment over time, RLE surgery is designed to provide lasting results, making it a strong long-term investment in your vision health.

Who Is a Candidate for Refractive Lens Exchange?

RLE is well suited to a specific group of patients. You may be an ideal candidate if you are:

  • Over the age of 50
  • Experiencing presbyopia or farsightedness
  • Affected by dry eyes
  • Not suitable for LASIK, SMILE, or other laser vision correction options
  • Living with early cataracts or wanting to prevent them

If you are considering RLE surgery, schedule a free consultation with the team at ScottHyver Visioncare. Our experienced surgeons will assess your eyes and advise whether this procedure is the right fit for you.

Types of Intraocular Lenses Used in RLE

The type of IOL selected will depend on your vision needs and lifestyle. Your surgeon will recommend the most appropriate option during your assessment.

IOL Type How It Works Best For Glasses Still Needed?
Monofocal IOL Provides sharp focus at a single distance (near or far) Patients who prioritise distance vision and are comfortable using reading glasses Possibly, for other distances
Extended Depth-of-Focus (EDOF) Delivers a continuous range of clear vision from near to far Active patients wanting broad visual range with minimal side effects Rarely
Multifocal IOL Uses advanced optics to distribute light across several focal points simultaneously Patients wanting full independence from glasses at all distances Typically no, but may cause halos or glare

For patients interested in correcting near vision specifically, monovision correction may also be worth discussing with your surgeon.

Recovery After RLE Surgery

Initial recovery from RLE typically takes around 3 to 4 days. During this period, it is normal to experience:

  • Temporarily blurred or fluctuating vision
  • Sensitivity to light and mild glare
  • Mild discomfort or a foreign body sensation in the eye

Full healing and visual stabilization generally occurs within 4 to 6 weeks as your eye adjusts to the new lens. To support a smooth recovery, it is important to:

  • Follow all post-operative care instructions from your surgeon
  • Use any prescribed eye drops as directed
  • Avoid strenuous physical activity during the initial recovery phase
  • Attend all scheduled follow-up appointments

According to the American Academy of Ophthalmology, lens replacement surgery carries a strong safety profile and high patient satisfaction rate, particularly among those with presbyopia and early cataracts.

RLE Results

The vast majority of patients who undergo refractive lensectomy with the Light Adjustable Lens experience high satisfaction after a single procedure. While uncommon, approximately 1% of patients may require a follow-up surgery to fine-tune results.

How Much Does Refractive Lens Exchange Cost?

RLE costs more than other laser vision correction procedures, such as LASIK. It is more expensive due to the advanced technology used in the lens implantation procedure. However, we offer 0% interest for 24 months or extended payment plans with interest for up to 60 months (subject to credit approval through CareCredit). We also have other options that could help you pay for eye surgery.

Frequently Asked Questions

Refractive lens exchange is a vision correction procedure that removes the eye’s natural lens and replaces it with an artificial intraocular lens (IOL). It uses the same surgical technique as cataract surgery, but is performed proactively on a clear or minimally affected lens, rather than waiting for a cataract to develop. The primary goal of RLE is to correct refractive errors such as farsightedness, nearsightedness, and presbyopia.

Yes. Because the natural lens is fully replaced with a synthetic IOL, the results of RLE surgery are considered permanent. The artificial lens cannot develop cataracts, meaning patients are also protected against future cataract-related vision loss.

RLE is most commonly recommended for adults over 50 who are not suitable candidates for laser eye surgery, such as LASIK or SMILE. It is particularly beneficial for those with presbyopia, farsightedness, dry eyes, or early cataract development. A comprehensive eye assessment with a qualified refractive surgeon is the best way to determine eligibility.

Most patients recover within 3 to 4 days and can return to everyday activities shortly after surgery. Full visual stabilization, as the eye fully adapts to the new intraocular lens, typically takes between 4 and 6 weeks. Your surgeon will provide tailored recovery guidance and monitor your progress at follow-up appointments.
